Medical Requirements 2023-2024

Freshmen & Transfer Students Only: All freshmen and transfer students should provide a completed physical with required immunizations to the school nurse by July 1st. The physical exam must have been completed within one year of entrance into 9th grade. iPads will not be mailed to students until a complete school physical signed by the parent/guardian is turned in. Please ensure this is completed as soon as possible. Students will be excluded from attending school until the school nurse has received the required Health Exam and Immunizations records. If you have questions, please call (630) 469-6500. Please note that the nurse will be available at school starting August 7, 2023.

Seniors Only: All seniors will need to provide proof of receiving their second Meningitis conjugate vaccine to the School Nurse by July 15th. Students will not be allowed to purchase parking passes until all requirements have been fulfilled. Students will be excluded from attending school until the School Nurse has received this information.

 

Medical Requirements for 9th Graders/Transfer Students

In order to process all freshman physicals in a timely manner the health office is requesting physical & immunizations be turned in by July 1st.

1st day exclusion will apply without completed requirements.

  • DHS State of IL. Certificate of Child Health Exam completed by M.D., D.O., A.P.N., or P.A. with signature and address stamp. Physicals must be dated on/after the first day of school of the previous school year to be accepted. Resources for obtaining a physical can be found on the health office website. Please note: Sports physicals are not accepted as the freshman physical.
  • Immunizations (if claiming religious objection submit required MD signed form with DHS physical. See below to access forms.)
    • Required Immunizations for freshman year: 
      • Tdap (1 dose)
      • Varicella (2 doses) 
      • DTap, DT, or Td (3 or more doses: last dose on/after 4th bday) 
      • IPV/OPV (3 or more doses: last dose given on/after 4th bday) 
      • Hepatitis B (3 doses) 
      • MMR (2 doses) 
      • Meningococcal (1 dose on/after the 11th bday)
  • Health History signed and dated by the parent 
  •  
  • MD completed action plans & medication forms for asthma, allergies, seizures, diabetes, and/or other health needs. Due by 1st day of school. To access forms, visit the Health Services page.
  • Dental Exam (See dental form for acceptable dates.)
