Glenbard South High School Athletic Forms Required Athletic Participation Forms The following forms must be up-to-date, completed, signed and either on file in the Athletic Office or with the coach in order for students to try-out for athletics. Athletes only need to fill out the forms once per school year. 1. GS Athletic Code, Registration Form, Concussion and Steriod Form: athleticform.pdf 2. IHSA Participation Physical Form (good for 13 months). Athletes can not try-out and participate without a current physical. IHSAphysical.pdf    (Freshman do not need this physical form if their Freshman physical has been completed within the year) The Athletic Fee of $130.00 will need to be paid before the first contest (checks payable to Glenbard South High School). Payments can be made on line at    http://www.glenbard87.org/sitepages/store/store All forms are also available in the main office or outside the Athletic Office.
